What is a Dual Special Needs Plan (D-SNP)?
A D-SNP is a type of Medicare Advantage plan that combines your Medicare and Medicaid coverage into one convenient plan. It’s tailored to meet the unique needs of people with both Medicare and Medicaid, helping coordinate your care and potentially offering extra benefits beyond Original Medicare.

With a D-SNP, you still receive all your Medicare Part A (hospital) and Part B (medical) benefits, and many plans include Medicare Part D (prescription drug) coverage. Your Medicaid benefits will continue as well, and the D-SNP helps coordinate between both programs.

Who Qualifies for a D-SNP?
You may qualify for a D-SNP if:

You have Medicare Part A and Part B, and

You receive full Medicaid benefits or qualify for one of the Medicare Savings Programs (such as QMB, SLMB, or QI).

You may qualify for SSBCI if you have a high risk for hospitalization and require intensive care coordination to manage chronic conditions such as Chronic Kidney Diseases, Chronic Lung Disorders, Cardiovascular Disorders, Chronic Heart Failure, or Diabetes.
